team are members of the primary healthcare team and provide nursing
care in the home for those patients who are not able to attend one
of the surgeries. |

The role of the Nurse Practioner involves: |

Assessing,
planning, implementing and evaluating patient care.

Prescribing
certain pharmaceutical items. |
| • |
Monitoring
standards of care.
|
| • |
Liaising
with other care agencies
(e.g. voluntary bodies, social services, palliative care, chiropody)
and local hospitals. |

Keeping
up to date with modern nursing practice and policies.

Training
for new nursing staff. |
